  • Quick to recognize issues that may impact the health and safety of your loved one



A Care Manager Will...

  • Complete a comprehensive in-home assessment to identify the individual’s and family’s needs

  • Determine appropriate services to meet those needs and assist with obtaining resources

  • Maintain regular communication with the individual, family, service providers, and healthcare professionals

  • Identify any needed changes to the care plan and supports based on the individual’s changing needs

NOTE: In order to be eligible for Care Management, the individual must be 60 years of age or older, have impairments in two activities of daily living, have complicated medical or behavioral impairments, and prefer to be cared for at home.
