There are times in our lives when we encounter the unique challenges of aging. Alamance ElderCare is here to help when you need...

  •  Information on services

  •  A caring person who will take the time to listen

  •  Assistance in putting the pieces of the puzzle together to find available services

  •  Help developing a plan and coping with life changes in a loved one

  •  Help to understand how the system works

A Care Manager is...

  • A professional with experience and compassion

  • An advocate for your loved one’s needs

  • Someone knowledgeable about community resources who continues to develop that knowledge

  • Quick to recognize issues that may impact the health and safety of your loved one

A Care Manager Will...

  • Determine appropriate services to meet those needs and assist with obtaining resources

  • Maintain regular communication with the individual, family, service providers, and healthcare professionals

  • Identify any needed changes to the care plan and supports based on the individual’s changing needs

NOTE: In order to be eligible for Care Management, the individual must be 60 years of age or older, have impairments in two activities of daily living, have complicated medical or behavioral impairments, and prefer to be cared for at home.
